Chronic Disease Self-Management Program Offered by ACHS & NCHC

Thank you - I got a lot from this course.” These are just some of the great comments made by past participants of the “Better Choices, Better Health” Chronic Disease Self-Management program, offered by Ammonoosuc Community Health Services in collaboration with the North Country Health Consortium.
This is a free, six-week, 2.5 hours per week workshop for caregivers of and those with chronic conditions such as diabetes, arthritis, high blood pressure, heart disease, COPD, chronic pain, and anxiety. These workshops are led by trained workshop facilitators.
This highly successful program was originally developed by Stanford University and is now a statewide initiative through a partnership with Southern New Hampshire Area Health Education Centers. The program provides a community setting where people with different chronic health problems get together for support and to find ways to deal with pain and fatigue, learn to handle stress, discover better nutrition and exercise choices, set and meet personal goals, improve communication with your doctor and more. This program is meant to compliment any ongoing clinical treatment of chronic disease.
