ACHS distinguishes itself from other medical practices by offering essential healthcare services at each of our 5 locations. Our patients have access to traditional healthcare services, as well as a wide range of other, related offerings.


Family Practice

Medical care for men, women, and children, including chronic disease management, preventive, exams, and acute illness care with around-the-clock availability.


Women’s Health

Preventive, acute, and chronic care that is specific to women, provided by healthcare professionals that understand a women’s needs and concerns.


Prenatal Care

Offered to all pregnant women, including those who are non-insured and Medicaid applicable. This service includes all prenatal care, delivery at either Littleton Regional Hospital or Cottage Hospital, and your 6-week checkup as well as post-pregnancy contraception counseling.


Chronic Care Management

Our care givers are educated in providing the very best Chronic Care management... Diabetes, heart disease, cardiovascular disease, asthma, depression. Come see us for your chronic disease management, as our national collaboration efforts set us apart.


Mental Health

The focus of our behavioral health services includes treatment for addiction, mental health issues, behavior modification, and problem solving therapy. All services are coordinated through your primary care provider allowing for comprehensive care management. ACHS recently recieved national recognition from HRSA for depression treatment outcomes.


Oral Health

Our special oral health program is available to Grafton County residents who are age 21 or older and income eligible. Please call our main office, 444-2464 to find out more about this program.



Special Programs...



Self-Pay/Uninsured Services

Insurance status and ability to pay are never barriers to care at ACHS. We will work within your income means to find a payment solution that works for both of us. Please call to arrange a confidential consultation regarding your self-pay status and our sliding fee scale.


Prescription Drug Assistance

Patients without prescription drug insurance who are income-qualified may receive long-term medications through the manufacturers Patient Assistance Program. Our staff at ACHS will help you to enroll in the program that is right for you— We also offer a 340B Discount Pharmacy program in partnership with Wells River Pharmacy in Wells River, VT.


WIC

Women, Infants & Children is a program that provides nutrition education and nutritious foods as well as referrals to help keep pregnant women, new mothers, infants and preschool children healthy and strong.


Partners in Health

The New Hampshire Partners in Health program is a community-based program designed to address the needs of families with children that have chronic health conditions.


Commodity Surplus Food Program

The CSFP program provides food to new mothers, children ages 5-6, and seniors 60 years or older who qualify on an income-sensitive basis. If you have needs, and feel you may qualify for this program please contact us for more details.


Pregnancy & Beyond

This home visiting program is available to eligible pregnant women and families with children 3 years and younger. The program provides support and education on child development and pregnancy, nutrition, and linkages to useful community resources.
